All players must complete the REQUIRED FORMS package by clicking on the link below, printing the forms and returning the completed AND signed forms to the club. The REQUIRED FORMS include the Waiver and Release of Liability, Player Waiver and Authorization for Medical Treatment.

Players NOT attending Holy Trinity Episcopal Academy should be aware that our coaches are affiliated with HTEA. If you have questions about what this means and how it may affect you, please ask. We will ask each player to complete section B of the FHSAA GA6 form as verification that you have been told about FHSAA Policy 22.
Club policy is NO FORMS, NO PARTICIPATION, NO EXCEPTIONS.
