2.  Emergency Medical Release & Liability Waiver   KS tournaments do require notarization, MO tournaments do not require notarization

3.  State Issued Copy of Birth Certificate (if you are a returning LUFC player, do not submit, only players new to LUFC)

Dual Roster Form  If your player has been asked to dual roster please complete your part of this form.  If your player is dual rostering, a WMSL Membership form needs to filled out for both teams.
