MAUI FOOTBALL COMBINE WALK-UP 2012 Registration

Athletes are required to submit the following forms, signed by at least one parent:

* Informed Consent Waiver

* Release of Liability of Waiver

IMPORTANT: Athletes/coach/parents, email address must be submitted to complete registration.

Note: This Form does not mean you have a spot at our Combine on June 15, you must still walk-up and sign-in at 4 p.m. on the day of the Combine.
